information about the medical worker providing care
methods, risks, contraindications and alternatives needed for informed consent
payment, termination and complaint conditions

Contract and payment

Terms are recorded in writing

The contract is made in writing and the patient receives a copy.
Payment is not consent to any intervention; informed consent is recorded separately.
Additional paid services require agreement and an amended contract or estimate.
Medical records are maintained regardless of payment method.
Website information is not a public offer; obligations are set by the signed contract.

Access to information or emergency care cannot be conditioned on purchasing an additional service. Service and payment terms are provided before the contract is signed.
